Western Products has made it easy for you to fill out and submit various craftsman forms. Click on the links below to access each form.

Pick Ticket/Return Ticket Form
Submit for any material picked up or returned for a Western Products warehouse location.
Service Request Form
Do you need help from our service department? To create a new service request, make an update to a service ticket or if you have a question on an existing service ticket, simply click on the button below and fill out the applicable information.
Completed Service Form
Have you completed a service job? Simply click on the button below to fill out Completed Service Submission form.
Window/Door Parts Order Request Form
Use this form to request parts for any window or door on your project. A request will be sent directly to James Frandson with the information you submitted.
Service Completion Certificate
Fill out this form for all completed service tickets.
Job Completion Certificate Form
Upon completion of the project, please fill out a job completion certificate. If there is something requiring service, indicate that the project is not substantially complete and then add the items to the punchlist.
Foreman Cut-Off/Draw Request

Craftsman Expense Report
Please fill out a form for each team member.
Job/Project Receipts Form
